Annual reports feature AI as a transformation pillar. Investor presentations describe AI-driven efficiency, AI-powered customer experience, AI-enabled risk management.

The production layer tells a different story.

In production, most financial institution AI programs consist of a small cluster of established models — typically fraud detection, credit scoring, and customer segmentation — that were deployed years ago and have been running with varying degrees of governance ever since. The most significant drop in this funnel occurs at the model risk review gate — not at the ideation stage, not at the pilot stage, and not due to technical failure. In my experience leading AI governance programs across Fortune 100 financial institutions and advising mid-market banks and credit unions, five governance blockers account for the majority of deployment failures. None of them are primarily technical.

No Accountable Owner

The model was built by data science, sponsored by a business unit, and reviewed by risk. No single person is accountable for its production performance. When the model risk committee asks "who owns this in production?" the answer is silence. The deployment pauses. It rarely restarts.

Validation Documentation Gap

The model works. The pilot results are strong. But the model risk team requires documented validation covering training data provenance, out-of-time testing, sensitivity analysis, and performance benchmarks. The data science team built a model, not a validation package. Closing the gap takes months.

The institution does not know how its regulator will interpret this model under SR 11-7, fair lending requirements, or emerging AI guidance. The institutions that have closed the deployment gap most effectively share a common operating structure: a CAIO or equivalent executive with explicit cross-functional authority, a governance architecture that treats compliance as a production input rather than a production gate, and a model registry that functions as the single source of truth for AI inventory, validation status, and production performance.

The Agentic AI Acceleration Problem

The deployment gap is about to get significantly worse before it gets better. Agentic AI — AI systems that take sequences of actions, make decisions across multi-step workflows, and interact with external systems without continuous human direction — is arriving in financial services at a pace that most governance programs are not designed to handle.

Traditional AI governance is designed for a model that takes an input, produces an output, and stops. Agentic AI takes an input, produces an output, acts on that output, evaluates the result, adjusts, acts again, and may interact with dozens of external systems before a human reviews what happened.
